About HekaHeart
HekaHeart is transforming cardiovascular care with our Virtual-First Heart Failure Clinic - helping cardiology practices and health systems manage their sickest, most complex patients. We pair an AI-enabled platform with a multidisciplinary team of cardiac-trained clinicians - APPs, RNs, PharmDs, & MAs - led by a board-certified Advanced Heart Failure specialist.
Our work is incredibly meaningful: every day we have the opportunity to help our patients live longer, healthier lives and spend more time with the people they care about.
Role Overview: Director of Engineering
As Director of Engineering, you’ll own the evolution of HekaHeart’s platform from a robust commercial ready software stack to an enterprise-grade, scalable system that supports tens of thousands of patients and dozens of clinics nationwide. You will recruit and oversee a small, exceptional engineering team as the company scales.
You’ll work closely with the CEO and Chief Medical Officer to translate clinical, operational, and commercial needs into elegant, correct solutions. This is an opportunity to build in healthcare — not for hype, but for real-world impact.
In this role it is important to us that you are curious and intellectually honest. You are not precious about your work. You iterate quickly, accept feedback gracefully, and know when to advocate for a change and when to move on. You understand that in a fast-moving startup, done is always better than perfect, except when perfect is the only acceptable standard.
What You’ll Do
- Scale our platform: Lead architecture and engineering strategy to support rapid growth while keeping systems modular, reliable, extremely well tested, and secure.
- Lead a lean team: Recruit, mentor, and manage a small, high-performing engineering org that can deliver with precision under resource constraints.
- Drive EMR integration: Build and maintain FHIR-based marketplace integrations with leading EMRs to enable data exchange and automated workflows (initial focus: eClinicalWorks, Athena, NextGen, Epic).
- Streamline clinician workflows: Collaborate with clinical ops to automate and optimize RPM and care management tasks through workflow design and lightweight AI/LLM tools.
- Build for reliability: Oversee security, compliance (HIPAA, SOC 2), and uptime standards.
- Partner across the business: Work directly with leadership, commercial, and clinical teams to ensure product roadmap aligns with business milestones.
Tech Stack
- Backend: Python, AWS (EC2, Lambda, Postgres/RDS, Cognito, API Gateway), Node.js
- Frontend: Welkin EHR UI (JSON API integration, iFrame/React components)
- Integrations: FHIR APIs, HL7 APIS, device + module + data vendor APIs
- Infrastructure: GitHub, Docker, Terraform
What We’re Looking For

- Strong understanding of FHIR, HL7, and healthcare data workflows
- Demonstrated success scaling a platform from commercial product → multi-site deployment
- Hands-on technical leadership — comfortable coding, reviewing, and shipping
- Ability to operate lean and prioritize intelligently in a growth-stage startup environment
Preferred
- Fully based in the Tri-State area
- Experience integrating with major EMRs (Athena, eCW, Epic, etc.)
- Familiarity with clinical workflows (RPM, clinical decision support, etc.)
- Track record of leveraging AI/LLM tools for workflow automation or data validation (not as a product feature)
- Prior startup or healthcare SaaS leadership experience
- Prior experience working within cardiology
- Excellent communication and collaboration skills with cross-functional teams
